Is your MILF physio so useless that she cant actually help or what? What about the swimming?
I have a similar issue with a loose shoulder joint from a climbing accident and subsequent wrestling injuries. Have not had surgery.
Give it time, start off with super low volume workouts, mostly mobility drills with low but increasing resistance. Then slowly increase volume until you can get into hypertrophic terrain. If you got an idea of what your 1 Rep Max load on a given shoulder excercise is; you should at least be getting to 30% of that 1RM for it to have a good hypertrophic effect. As long as you dont feel pain, do that weight for a number of reps that gets you close to failure.
Number of workouts per week, sets per workout etc. depends on your progression and the excercises you're doing. Just giving you some pointers. See a good trainer.

Nawh my dream milf has actually done a spectacular job so far; not too far off from full range of motion at this point. Got cleared for weighted back lifts w progressive overload, weighted shoulder lifts (but still lowweight), and most recently pushups too. Have been swimming couple times a week; usually only for like 20-30min at a time though.
Really just concerned w longevity I guess; I wanna be able to keep going harder as I age

I’m doing resistance band stuff for pt haha and yeah I’ll give you that one my shoulders are pathetic at best rn lol

Most resistance band training is in the realm of recovery. Tightening and strengthening loosened tendons and minor muscles.
Those excercises usually dont provide enough resistance for you to see actual gains. Keep doing them progressively harder until you can pick up the real weights. It's critical that you dont go too fast, or you'll fuck it up again.
